Patient Rights
As a patient/client of Progressive Care Network you have the right:

to healthcare that meets professional standards, and is performed by persons who are competent and qualified to carry out the services for which they are responsible.

to be treated with courtesy and respect, to receive service without regard to race, color, creed, national origin, sex, handicap, age or sexual preference.

to receive information about the company’s products and services, the reasons for treatment and the risks of therapy in terms you can understand so you can give informed consent.

to make decisions about your care and a responsibility to participate in and carry out your plan of care. YOU HAVE A RIGHT TO EXECUTE AN ADVANCE DIRECTIVE.

to refuse treatment to the extent permitted by law and to be informed of the health consequences of such refusal.

to be taught about your therapy and/or equipment so that you or your caregiver understand the therapy and/or the equipment and your responsibility and involvement in your care.

to confidential treatment of all records and communications, to approve the release of these records to any individual or organization outside of our company, except as required by law.

to receive response to any reasonable request for services.

to information on charges for our company’s services regardless of source of payment.

to know when your physician proposes to engage in human experimentation affecting your care or treatment. You have the right to refuse to participate in such research projects.

to voice your grievance and recommend change in policies and services. If you have a complaint, please call the company at (317)596-2805, or 1 (800)921-6496. Teresa Goldman or her designee will review and resolve your complaint.

to choose your provider of home care services. When necessary, we will provide assistance in finding and transferring your services to another home care provider.

to have appropriate assessment and management of pain.
(A patient’s rights as stated above may be denied for good cause only by the patient’s physician.)
Patient Responsibilities
You, and when appropriate, your family, also have certain responsibilities as a patient receiving homecare services.
It is your responsibility:

to be taught the necessary steps to administer your therapy or use your equipment and to adhere to those procedures

to be compliant with your therapy regimen, schedule, and/or equipment maintenance.

to provide Progressive Care Network with accurate and complete information about your medical condition and history.
-
to notify Progressive Care Network of any changes in your condition or therapy, and any difficulties with your therapy, equipment or supplies.

to notify Progressive Care Network if you will be unavailable for visits, move, travel, or are hospitalized.

to inform Progressive Care Network of appointments with physicians.

to assure that the financial obligations of your healthcare services are fulfilled as promptly as possible.

to be respectful of the property of Progressive Care Network and to treat Progressive Care Network associates courteously.

to assure your home meets the safety requirements necessary for the safe use of the equipment provided to you; and that the proper community resources are notified of any emergency response needs you may have related to the care or equipment provided in your home.

to inform Progressive Care Network of any advance directives you may have regarding your care.
